Hancock Park and DaVita Deliver On-Site Skilled Nursing Facility Dialysis

Life-sustaining kidney care available to Hancock Park Rehabilitation & Nursing Center residents

 

June 3, 2024: Hancock Park and DaVita Skilled Nursing Facility (SNF) Dialysis are proud to bring an on-site, kidney care program to Quincy, MA.

The on-site dialysis center is exclusively available to short-term rehabilitation patients and long-term care residents of Hancock Park.

DaVita’s highly trained and dedicated care team members deliver on-site care to residents of Hancock Park, creating a more comfortable and convenient experience for many patients and their care partners.

 

“The opening of our in-house dialysis den is not just a milestone for Hancock Park, but an opportunity to improve the patient experience and clinical outcomes for those facing the challenges of kidney disease”, said Kevin Morris, President and CEO of BaneCare Management, the operating manager of Hancock Park. “It's a testament to our commitment to our patients by removing the obstacles they face when trying to a maintain a dialysis schedule at the same time as receiving inpatient nursing care and rehabilitation.”

 

Hancock Park provides 24-hour nursing services; occupational, physical and speech therapies; and on-site hemodialysis — reducing the discomfort, inconvenience, and cost of frequent transportation for off-site treatment.

 

This program is also projected to reduce the total cost of care to payors like Medicare, as well as reduce hospital readmission events. SNFs within the DaVita Post-Acute network offer a streamlined discharge planning process to hospitals by eliminating the need for referrals for transportation and community dialysis appointments. Patients may return to their community dialysis center and nephrologist when ready, and their community nephrologist is invited to follow them while at Hancock Park.
